At Southern Aged Care Placement Services (SACPS), we are passionate about the future care of our elderly citizens and empathetic to both their situation and their family during this difficult, stressful and emotional time.
We are committed to assisting the elderly and their family to access the right services to meet the individual needs of our elderly clients, regardless of what those needs are. Whatever the needs, you can rest assured that we will provide a professional service to seek and organise suitable Aged Care placement/Home Care Services, thus reducing the amount of stress within the family.
SACPS understands that navigating the aged care system can be confusing and frustrating for both the client and their family. We understand that the process of finding suitable aged care placement/home care services can be stressful and time consuming. SACPS Consultants are flexible and will meet clients in their own home at a time suitable to the client and their family (within reason).
No two-elderly people are the same. As a result, we assess each person as an individual and tailor their care needs accordingly. We at SACPS believe in a holistic approach.
A holistic approach incorporates not only ones’ physical well-being but also engages the person as a whole. This simply means the individuals Physical, Emotional, Mental and Spiritual well-being.
To ensure that SACPS can organise appropriate services to meet our client needs, a SACPS consultant will assess each client to establish what care needs are required. Our assessment will include input from other health professionals involved in the client’s care. For example, the individuals GP and/or other medical doctors/specialists, nursing staff, Aged Care Assessment Team, Social Workers, Physios, etc. It is equally important to have the clients’ family members involved and to contribute and participate in this assessment process.
In consultation with the client and their family, we at SACPS liaise with other members of the health care team so that we can develop, organise and facilitate care services that are individualised to the clients’ care needs. SACPS will provide support during this transition and once services have been implemented and commenced, we will conduct a follow up to ensure that the client and their family are happy with the services being provided.
The Products & Services we offer include the following:
SACPS will complete and lodge all documentation required to access Home Care Services or for the purposes of Aged Care Placement.
SACPS will develop, organise and facilitate suitable Home Care Services to meet the Clients assessed care needs, in consultation with family and as assessed by the Aged Care Assessment Team, SACPS consultants and other health professionals.
SACPS will locate suitable Aged Care Placement to meet the Clients care needs, as practicable and in consultation with family. We will do the running around for you.
SACPS will escort family and the Client (whenever possible) to view prospective Aged Care Facilities or available beds as offered by a facility.
SACPS will assist with the transfer of the Client from their current home or dwelling into the approved Aged Care Facility as required and by prior arrangement.
